PrimeCare Health: 2022-2023

PrimeCare is a Federally Qualified Health Center (FQHC) delivering high-quality, low-cost health care to Chicago’s west-side communities. As an FQHC, PrimeCare provides a myriad of services including, but not limited to primary care, dental, behavioral health, medication assisted treatment, case management, and access and enrollment. Last year, PrimeCare served 27,915 patients. Throughout PrimeCare’s 31 years of existence many new adjustments and programs have had to be made and adapted to fit the communities’ needs.

A grant from the Washington Square Health Foundation allowed PrimeCare to develop and innovative program called the Integrative Medicine Program. This program combines evidence-based primary care and behavioral therapies with complementary approaches to advance optimal health outcomes and lifestyle changes for patients. With high rates of chronic illness such as heart disease, diabetes, and hypertension throughout our service area, the Integrative Medicine Program focuses on reducing obesity
throughout our service area to prevent chronic illnesses from occurring in both children and adults.

The children’s Integrative Medicine Program is age-appropriate and engages the entire family by making lifestyle changes, for example, adding fresh produce to their diet, learning to cook healthy foods, incorporating physical activity into their daily routines. The family is led by a primary care provider
and a behavioral health consultant (BHC) to make these changes and are given new tasks at each follow-up appointment to advance their health outcomes based on progress. The adult’s Integrative Medicine Program is a project where the patient creates a custom plan with their provider to get to a healthy BMI, and then attends regular meetings with their provider to adjust the plan based on progress. The adult is also assigned a BHC who provides on-going support for achieving and maintaining positive healthy
lifestyle changes. The partnership with Washington Square Health Foundation is allowing PrimeCare the opportunity to help patients reach a healthy weight and reduce risk of chronic illness.
