- Provide General Information About Your Project
This information is specified in the online application. - Prepare a Brief Project Proposal
Identify the type of project for which you are seeking support by reading the descriptions below. Then, depending on the type of project, a series of questions must be answered and uploaded to the online application that will guide you in the preparation of a brief project proposal. If a project is both program and research all questions requested for both types of projects must be answered. If your project involves equipment, and is either a program and/or research request, you must complete the required equipment questions as well.- Program Support
These proposals are to support a broad range of NEW program initiatives including (but not limited to) a new health education program, a new type of health care delivery system, a new treatment program, a new medical education program, an institutional change such as a new Emergency Department triage system, a new use of computer technology such as for clinician training or evaluation, or a new health service such as a dental clinic within a community health center. These projects are often being done as pilots or as initial phases of larger projects.If your project matches this description, please answer all the questions in Section A and upload them to the appropriate section in the online application. - Research Support
These proposals are for support of research activities–usually clinical research, health services research or health education program evaluation. All proposals seeking funding for equipment that will be used in a research project should be submitted as this type of project.If your project matches this description, please answer all the questions in Section B and upload them to the appropriate section in the online application. - Equipment Only
These proposals are generally for specific pieces of patient care-related equipment such as a neonatal transport unit, a dental x-ray machine, or specialized surgical or laboratory equipment. There are several exclusions:- Requests for resources to purchase computer equipment or information systems should be submitted as a program support or research support grant as appropriate. The Foundation does not provide funding for computers to be used for administrative/operating purposes.
- All requests for equipment that will be used in a research project should be submitted as a research support grant.

Anyone applying for a grant, must complete the following form and upload it to the appropriate section in the online application: - Upload Appropriate Documentation
Do not forget to upload a copy of the most recent annual report, audited financial statement and official documents certifying the not-for-profit status (IRS 501(c)(3) determination letter) of the sponsoring organization, association or institution.For Research Support grants, IRB approval, if required, must also be submitted to the Foundation office with the completed application. - Verify Application is Complete

Grant submission deadlines (application must be uploaded by 11:59 pm) are as follows: December 1, for the January\February Grant Committee Meeting or June 1 for the July\ August Grant Committee meeting.
